Please fill out the following form

Is your K-9/Feline/Exotic pet's up to date on their Rabies and Bordetella vaccine? No need for verification; I approve that A Trusted Friend is not liable for any illnesses incurred during my pet's stay!
Is your pet potty-trained? (No marking in new enviornments)
Would you like a go-home bath during their boarding stay? ($25-$60) *Please select no for grooming appointments
Does your pet get along with other animals, Big or Small? (other dogs)
Does your pet attempt to jump fences or dig to escape?
Does your pet respect kittens or cats?

Do you authorize any and all injuries that require veterinary care be covered by A Trusted Friend? (Pre-existing conditions are excluded)
